Creating a Blank Google Forms
- First, you open the Google forms site https://www.google.com/forms/about/ , there will be 4 options, namely Docs, Sheet, Slide, Forms, you select Forms, and click the blue button that says " Go to Google Forms ".
- There you will see several Preset Forms, such as Party Invitation, Contact Information, RSVP, Event Registration because we are here to create an Online Exam, so we choose " Blank Form ".
Creating Online Exam Login Page
- In the words " Untitled Page " You give a free title according to what you plan, for example, " Online MTK Class XII Semester 2 Exam "
- For the " Form Description " Section, it is actually optional to fill out, but most of the Form Descriptions are filled with School addresses.
- Then, you fill in the " Question " column with " Student name ".
- And for " Multiple Choice " you replace it with " Short answer "
- For the Required Option, you just activate it, so that students don't forget to enter their names.

Creating Questions and Entering Answer Keys
1. Next, you click to add the section on the right side, if it's not clear, you can see the image below.
2. Next, you fill in the Form Title and form description as usual.
3. Then, you click the add question button on the right side with the (+) symbol.
4. Then you enter the question and fill in option 1 to option 5 for the answer.
5. Enter the answer key, in order to choose which answer is correct.
6. Rate the question, for example, 10 points/question or 5 points/question. For a sample image, see the image below.
7. Then we set the form, by clicking the settings button on the top right.
8. then, click the Quiz section, and check the " Make this a quiz " section
9. For other settings, please set according to your own will.
NOTES :If the button to enter the Answer Key does not appear, then you must activate " Make this a quiz " first.
